We provide treatments for referred patients from General Dental Practitioners. If you are a dentist and wish to refer a patient then the referral form can be downloaded from below. This can be e-mailed or printed out and sent via post or fax. If you are a patient and already have a dentist then you can be referred from your own dentist to MAC Dental. We will always send back referred patients after the requested treatment has been completed and under no circumstances keep the patient.
